In compliance with the legal obligation for the athlete to ensure their physical capacity (cfr. Art. 40.o, n.os 1 and 2 of the Basic Law of Physical Activity and Sport, approved by the law n.o 5/2007, 16th january, and Order n.o 11318/2009 of the Office of the Secretary of State for Youth and Sports), I declare and confirm that the Participant has no contraindications for engaging in physical and sports activities, including those related to the Elite Training Camp.
On my behalf and that of my representative, as identified above, I freely, consciously, and informedly consent to the companies that are part of the Grupo Benfica* collecting and processing the personal data provided above for internal procedures (such as issuing sports insurance and accommodation), for possible visa issuance, obtaining entry permission into the country, complying with judicial/police orders, for medical treatments, booking travel and issuing the respective tickets, for managing the relationship with the institution, and for marketing and advertising actions. On behalf of my represented individual, as identified above, I freely, consciously, and informedly consent to any of the companies that are part of the Grupo Benfica* capturing, recording, and processing images and sound of the Participant during the activities of the Elite Training Camp, regardless of the medium, and using such images and sound for transmission, reproduction, publication, promotion, adaptation, through any means they deem appropriate, to promote the Elite Training Camp and the Benfica brand. On behalf of myself and my represented individual, as identified above, I freely, consciously, and informedly consent to the companies that are part of the Grupo Benfica * sharing the personal data provided above with third parties - specifically, courts, police, SEF (Foreigners and Borders Service), hospitals, travel agencies (for any legal, administrative, and/or procedural purposes), as well as partners (for commercial purposes). *Grupo Benfica: Currently, Sport Lisboa e Benfica; Sport Lisboa e Benfica – Futebol, SAD; Sport Lisboa e Benfica, SGPS, S.A.; Sport Lisboa e Benfica – Multimédia, S.A.; Benfica Estádio – Construção e Gestão de Estádios, S.A.; Parque do Benfica – Sociedade Imobiliária, S.A.; Clínica do SLB, Lda; Benfica TV, S.A.; Sport Lisboa e Benfica – Seguros, Mediação de Seguros, Lda; Fundação Benfica.
